Altar of Fangs is a three-boss dungeon in WoW Midnight Patch 12.1 and part of the Midnight Season 2 Mythic+ rotation. It is located inside the Vaults of Atal'Utek beneath the Coiled Isle and contains Rav'i, The Writhing Coil and Zul'jan.
The dungeon combines poison dispels, mandatory interrupts, controlled movement and several mechanics that become much more dangerous in Mythic+. Rav'i is built around Carrion Piles and Fresh Meat, The Writhing Coil must be physically pulled apart during Death Rattle, and Zul'jan requires the group to intercept Ritual of the Fang beams and deliberately clear Ritual Venom.
This guide covers the live dungeon entrance, route structure, important trash abilities, all three boss strategies, Mythic differences, current Season 2 loot and Mythic+ rewards.

Altar of Fangs +10 Teleport
Players who complete Altar of Fangs at Mythic+ 10 or higher within the timer can unlock Path of Venomous Evolution, allowing direct teleportation to the dungeon entrance from the General tab of the Spellbook.
Altar of Fangs Quest and Campaign Access
The dungeon is directly connected to the Patch 12.1 campaign through The Vaults of Atal'Utek: Altar of Fangs.
The quest asks players to:
- check on the forces inside the Vaults of Atal'Utek;
- meet Lady Liadrin;
- clear Altar of Fangs;
- confront Zul'jan;
- continue the story surrounding the Fang of Ula'tek.
If you are completing the campaign quest rather than simply running a Mythic+ key, follow the NPC objectives in order instead of immediately queueing and killing the final boss. The dungeon forms the opening chapter of the Battle for Atal'Utek storyline before the events leading into The Venomous Abyss.

The exact Mythic+ pull pattern can vary with group composition, key level and weekly dungeon conditions, but the dungeon has several fixed progression gates that every route must respect.
| Section | Important Route Rule |
|---|---|
| Opening area | Activate all required Totems before progressing to Rav'i. |
| Before Rav'i | Control Primal Serpents, Twinfang Harrowers and Ravenous Descendants. |
| After Rav'i | Stop Evolve from High Evolutionists and destroy Empowered High Evolutionist shields quickly. |
| The Writhing Coil | Second boss and major cooldown opportunity during Uncoil. |
| After The Writhing Coil | The Ascendant Serpent elite must be defeated to progress. |
| Final section | Interrupt Mass Envenom from Ula'tek's Chosen and preserve defensives for Zul'jan. |
| Zul'jan | Final boss; pre-assign Ritual of the Fang beam positions. |
Altar of Fangs MDT Route
There is no reason to hard-code one permanent MDT pull order into an evergreen dungeon guide. Pack combinations can change with key level, composition and tuning.
The stable route principles are:
- complete all mandatory Totem interactions before Rav'i;
- do not combine dangerous caster packs unless your group has enough stops;
- save AoE cooldowns for valuable large pulls and The Writhing Coil's Uncoil phase;
- respect the mandatory Ascendant Serpent after boss two;
- reach Zul'jan with enough time to play the beam mechanic rather than forcing an unsafe final pull.
Cursed Bones in Altar of Fangs
Cursed Bones are clickable objects found inside Altar of Fangs.
Live community testing reports interactions involving Soothed Amani Spirits and Twisted Curse effects. Because this interaction is not part of the core Dungeon Journal boss strategy and its optimal use can depend on the current live behavior, treat Cursed Bones as optional advanced route technology rather than something required for completing the dungeon.

Rav'i Boss Guide
Rav'i is fought in a pit filled with Carrion Piles. The encounter is largely determined by which pile is closest when Rav'i runs out of energy and begins feeding.
Ravenous Stomp and Fresh Meat
Ravenous Stomp deals party-wide damage and causes stalactites to fall into the arena.
Avoid the impact circles. Falling rocks can skewer flying Twinfangs, causing Fresh Meat to fall onto nearby Carrion Piles.
The tank should immediately identify which Carrion Piles received Fresh Meat and keep Rav'i positioned so the nearest pile during the next feeding event is preferably a normal one.
Ssscavenging vs Feeding Frenzy
When Rav'i runs out of energy, she moves to the nearest Carrion Pile and gains an absorb shield.
If the pile contains no Fresh Meat, she uses Ssscavenging and applies Carrion Burst every three seconds until the shield is destroyed.
If the pile contains Fresh Meat, she enters Feeding Frenzy and applies Carrion Burst approximately twice as quickly.
The DPS rule is simple: switch immediately and destroy the absorb shield. Every additional Carrion Burst creates more stacking group damage.
Messy Eater on Mythic
On Mythic and Mythic+, Rav'i throws chunks of rotting meat around the pit while feeding.
Each chunk must be handled. A chunk that is not caught causes another Carrion Burst, adding unnecessary stacks to the entire party.
Assign players to the visible landing zones rather than allowing every member of the group to chase the same chunk.
Triple Shot
Triple Shot targets multiple players with venom and creates damage around the affected targets.
Spread before the hits land and avoid overlapping the circles.
Regurgitate
Rav'i fires three lines of acid toward a player.
Being hit deals heavy Nature damage and significantly reduces both movement speed and damage dealt. Avoid the waves first; if someone is hit and the resulting effect is dispellable by your composition, remove it quickly.
Hydrastrike
Hydrastrike is Rav'i's primary tank attack. Keep the boss pointed away from other players and combine defensive cooldowns with periods where Scent of Blood or feeding pressure increases the tank's incoming damage.
Rav'i Role Summary
Tank: keep Rav'i away from Fresh Meat Carrion Piles and face Hydrastrike away from the group.
Healer: prepare for escalating Carrion Burst stacks and stabilize Triple Shot targets before feeding phases.
DPS: instantly switch to the feeding shield, spread Triple Shot and handle Mythic meat impacts.
The Writhing Coil Boss Guide
The Writhing Coil alternates between one large serpent mass and an Uncoiled form consisting of five separate snakes.
The encounter's most important rule is that Death Rattle cannot simply be healed indefinitely. The party must use Orweyna's Vine Grip correctly to pull the boss apart.
Toxic Atrophy
The Writhing Coil prepares three Toxic Atrophy casts in succession.
Each successful cast reduces damage done and movement speed and can stack. Assign interrupts before the pull so three consecutive casts do not catch the group without a stop.
Death Rattle and Vine Grip
Death Rattle deals increasingly dangerous party-wide Physical damage.
Orweyna applies Vine Grip. Players attached to the Coil must move at least approximately 10 yards away from it.
Move immediately. Delaying the pull only increases Death Rattle stacks and wastes the encounter's best damage phase.
Uncoil
When the Vine Grip requirement is satisfied, The Writhing Coil splits into five Uncoiled Writhes.
This is a major AoE damage opportunity. Damage dealt to the snakes persists after they recombine through Assimilation.
Use:
- AoE offensive cooldowns;
- grouping and crowd control;
- interrupts on Toxic Atrophy;
- priority damage on dangerous or low-health snakes.
Spiteful Hunt on Mythic
On Mythic and Mythic+, individual Uncoiled Writhes use Spiteful Hunt and fixate players.
If a snake reaches its target, it applies stacking Spiteful Venom. Kite, stun and control the fixated snakes while maintaining AoE damage.
Vindictive Onslaught
The main Coil uses a sequence containing:
- Burrowing Charge — move out of the boss's underground charge path;
- Venom Jet — avoid the frontal cone;
- Tail Scythe — heavy Physical damage on the current tank.
Synchronized Venom
Synchronized Venom applies group-wide Nature damage and a long periodic effect. Healers should stabilize the party before Death Rattle or Uncoil overlaps rather than trying to recover after the next major event has already started.
Zul'jan Boss Guide
Zul'jan is the final boss of Altar of Fangs and the most coordination-heavy encounter in the dungeon.
The fight revolves around Ritual of the Fang, Ritual Venom and Zul'jan's Physical attacks that trigger Bloodletting.
Ritual of the Fang
Snake heads channel four ancient beams toward Zul'jan for eight seconds.
The group should intercept all four beams. Any beam allowed to reach Zul'jan contributes to Fang Empowered, causing increasing party-wide Nature damage.
Pre-assign starting positions before the pull so players are not all running toward the same beam.
Ritual Venom
Intercepting a beam applies Ritual Venom.
The debuff lasts long enough to be handled deliberately, but it must not be ignored. On Mythic it can stack, making repeated beam assignments increasingly dangerous.
Bloodletting and Boneslicer
Zul'jan's Physical abilities trigger Bloodletting. Bloodletting removes Ritual Venom stacks but also creates damaging blood pools.
Boneslicer is particularly useful for this interaction. Zul'jan throws an axe in a line through a targeted player. Players carrying Ritual Venom can deliberately position in the assigned Boneslicer line so the hit removes their Venom.
A single successful Boneslicer interaction can remove all Ritual Venom stacks from the player.
Do not turn this into random line soaking. Pre-assign which players need the clear and keep everyone else out of the path.
Axegrinder
Axegrinder launches spinning axes that ricochet around the room.
The axes lose power as they travel but remain dangerous. Preserve movement space by placing Bloodletting pools away from the center whenever possible.
Chop Down
Chop Down strikes the current tank twice for heavy Physical damage.
Use active mitigation before the sequence rather than reacting after the first hit.
Zul'jan Role Summary
Tank: use mitigation for Chop Down, keep Zul'jan stable and do not place unnecessary Bloodletting pools in the center.
Healer: track players carrying Ritual Venom and prepare for increasing Fang Empowered pressure if a beam is missed.
DPS: intercept assigned beams, use Boneslicer correctly to remove Ritual Venom and keep moving for Axegrinder.
Heroic vs Mythic Altar of Fangs
| Boss | Important Mythic Difference |
|---|---|
| Rav'i | Messy Eater meat chunks must be caught or they trigger additional Carrion Burst. |
| The Writhing Coil | Uncoiled Writhes use Spiteful Hunt and fixate players. |
| Zul'jan | Ritual Venom can stack, making beam and Boneslicer assignments more important. |

Notable Altar of Fangs Trinkets
Coiled Fangstone
Coiled Fangstone is a Strength trinket from Rav'i. Its active effect releases three Envenomed Bites around the target, with additional scaling when several enemies are nearby. That makes it particularly relevant to dungeon and cleave-focused builds.
Vile Vial of Volatile Venom
Vile Vial of Volatile Venom grants Agility, Strength or Intellect depending on specialization. Its active effect temporarily grants a large random secondary-stat bonus before later reducing a random secondary stat.
Knot of Writhing Serpents
Knot of Writhing Serpents is an Intellect trinket from The Writhing Coil. Harmful spells can trigger a serpent attack that deals Nature damage and gains value against multiple enemies.
Tattered Amani War Banner
Tattered Amani War Banner is an Agility/Strength trinket from Zul'jan. The banner creates an area that grants your highest secondary stat while fighting inside it, and defeating an enemy while the banner is active can extend its duration.
Do not treat dungeon trinkets as universally Best in Slot. Their value depends on specialization, key level, target count and current class tuning.
Altar of Fangs Mythic+ Loot Levels

| +10 and higher | 311 — Hero 3/6 | 318 — Myth 1/6 |
+10 is the main Season 2 gearing breakpoint: normal end-of-run gear and Dungeon Great Vault item levels both reach their standard caps there. +9 is also important because Myth Mistcrests begin at that level.

Great Vault and Nebulous Voidcore
An eligible Altar of Fangs Mythic+ completion contributes to the Dungeon row of the Great Vault.
Dungeon choices unlock after 1, 4 and 8 qualifying dungeon completions. Their quality is determined by your highest, fourth-highest and eighth-highest runs.
Altar of Fangs is also eligible for the Season 2 Nebulous Voidcore bonus-loot system. After an eligible Mythic+ completion, a Voidcore can provide an additional class-specific loot opportunity at the corresponding Vault-equivalent level.
If you are choosing between multiple weekly activities, use the Midnight Season 2 Weekly Checklist.
